No, it's the return crank, the little wobbly bit that works like an eccentric to move the expansion link back and forth. On a real engine it attaches to the extended shaft on the main crank on the rear wheels with a great big bolt that tightens it onto the shaft. On the roundhouse engines it attache...

I don't really see that it's all that radical, stopping the engine every 15 minutes or so to pump some more water in there, how is this different than keeping a gas fired locomotive running continuously?
